Health and Wellness Coaches may collaborate with Veterans and Veterans' primary care and specialty care treatment providers to develop goals designed to improve health and well-being, reduce impact from chronic illness and lessen the likelihood of admission to the emergency department, inpatient care or long-term care. These duties include but are not limited to:
